These Sweepstakes Terms and Conditions (“Official Rules”) govern the Tinnitus Treatment Seekers Survey Sweepstakes. This sweepstakes is sponsored by Neuromod USA Inc.
1. ELIGIBILITY
The Sweepstakes is open only to legal residents of the United States of America, the United Kingdom, and European Union who are 18-years-old at the time of entry. Employees, officers, and directors of Sponsor, its affiliates, subsidiaries, advertising and promotion agencies, and their immediate family members or persons living in the same household are not eligible to participate.
The Sweepstakes is void where prohibited by law.

3. SWEEPSTAKES PERIOD
The Sweepstakes begins on Wednesday, November 12, 2025 at 9:00am US Eastern Time and ends on Monday, November 24, 2025 at 9:00am US Eastern Time. Entries received outside of this period will not be eligible.
4. HOW TO ENTER
To enter, complete and submit the Tinnitus Treatment Seekers Survey, including your email address after the final survey question has been completed. Limited to one entry per person. Entries that are incomplete, late, or otherwise not in compliance with these Official Rules may be disqualified at Neuromod’s discretion.
No purchase or payment is necessary to enter or win.
5. PRIZE(S)
The prize(s) and approximate retail value (“ARV”) are as follows:
- 1st Prize: $1000 Amazon Gift Card
- 2nd Prize: $500 Amazon Gift Card
- 3rd Prize $250 Amazon Gift Card
Prizes are non-transferable and no substitution or cash redemption is permitted, except at Sponsor’s sole discretion.
6. WINNER SELECTION AND NOTIFICATION
Winner(s) will be selected in a random drawing conducted on or about November 24, 2025 from among all eligible entries received.
Potential winners will be notified by email within 3 days after the drawing. If a potential winner cannot be contacted or fails to respond within 24 hours of notification, the prize may be forfeited and an alternate winner selected.
7. PUBLICITY RELEASE
Except where prohibited, participation in the Sweepstakes constitutes the winner’s consent to Sponsor’s use of winner’s name, likeness, voice, opinions, and city/state of residence for promotional purposes in any media, worldwide, without additional payment or consideration.
8. GENERAL CONDITIONS
Sponsor reserves the right to cancel, suspend, or modify the Sweepstakes if fraud, technical failures, or any other factor beyond Sponsor’s control impairs the integrity or proper functioning of the Sweepstakes.
Sponsor’s decisions are final and binding in all matters related to the Sweepstakes.
9. RELEASE AND LIMITATION OF LIABILITY
By participating, entrants agree to release and hold harmless the Sponsor and its affiliates, subsidiaries, and agents from any claims, damages, or liability arising from participation in the Sweepstakes or receipt or use of any prize. Sponsor is not responsible for lost, late, incomplete, or misdirected entries or for any technical issues that may affect entry.
10. GOVERNING LAW
All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules shall be governed by the laws of sweepstakes participants’ local governing body, without giving effect to any choice of law or conflict of law rules.
